Waiver of Liability for Youth Sports Event My son/daughter and I are voluntarily participating in the youth sports event being held by (theDOBO) in (July of 2024). I acknowledge that participating in this event carries a risk of injury and damage to personal property. I certify that I am 18 years of age or older and have read and understand the terms of this waiver.
